Filling out Indemnification Agreement for Sole Proprietorship/Individual

 

If the company is a Sole Proprietorship/Individual then the owner (proprietor)/individual must sign the Indemnification Agreement as follows:

 

The owner/individual must sign as "Indemnitor," and must sign again (below) providing their personal indemnity.  If the owner/individual is married, then the spouse's signature may also be required as indemnitor.

 

Example:  Jason Doore DBA AAA Auto Sales (Owner is Jason Doore.  Spouse is Jane Doore.)
